Default taper: FATAL syncpipe_get: r: unexpected EOF

Hi - I'm new to amanda and am having problems with one of our backup heads - nightly config has been running absolutely fine since but the monthly archive ran over the weekend (different config) - dumped to disk as expected but won't flush to tape - last months was fine.

4 brand new LT02 tapes for this particular backup - freshly labelled.

The following errors are included in the email after I manually run amflush.

    Code:
The next 4 tapes Amanda expects to use are: a new tape, a new tape, a new tape, a new tape.
    The next 4 new tapes already labelled are: ibcM34, ibcM35, ibcM36, ibcM37.

The changer script appears to be timing out from the drive not becoming ready.

'Drive not ready after 120 seconds, rewind said "/dev/nst0: Input/output error"'

Any ideas why the drive is giving an input/output error upon rewind?
